Please arrive with your insurance cards and any referral forms required by your insurance company. If your insurance provider requires you to present an authorizing letter, please bring this with you as well. We participate in most major insurance plans and will be pleased to bill your insurance carrier on your behalf. Copays and deductibles are the responsibility of the patient. Because insurance plans vary greatly, it is your responsibility to contact your insurance carrier to determine eligibility, copay, deductible and referral requirements prior to your appointment.
Copays and Deductibles
If a copay is required by your insurance, it is to be paid prior to seeing our provider. We are required by your insurance to collect the copay at the time of service. Remember, insurance companies often apply different copay amounts for specialists. Please come prepared to pay this amount. Many insurance policies have a yearly deductible that must be met by the patient before the insurance will pay their percentage. Any unmet deductibles must be paid at the time of service for our surgical appointments.
Referrals
Some health insurance policies allow you to make an appointment directly with a specialist while others require a referral from your primary care physician. Other insurance companies only require a note from your primary care physician. If you have questions regarding referral requirements, please contact your insurance carrier. Individual policies vary greatly.

We highly recommend that you contact your insurance company to confirm a referral is not required and that Mountaintop Dermatology is in network with your insurance.
