This page will help you better prepare for your visit and give you information about what to expect.  Being prepared for your visit with us at Mountaintop Dermatology will ensure a smooth check-in and prevent any unnecessary delays. 

What are the Practice Hours?

What do I need to bring?

Please arrive with your insurance cards and any referral forms required by your insurance company. If your insurance provider requires you to present an authorizing letter, please bring this with you as well. If you are unsure of exactly what will be required, please call your insurance company before your appointment.  For a list of insurance plans we participate in, please see our Insurance Information.

In addition to your insurance information please make sure you bring the following to your first appointment:

• A completed copy of our new patient visit form. See Downloadable Forms for more information.
• Co-pay or co-insurance for the visit.
• The name and phone number of your pharmacy.
• A list of the medications you currently use, including any topical ointments or cream.  
• For minors, a parent or legal guardian must be present for the first consultation. If you are a legal guardian, please bring proof of guardianship.

If you need to cancel your appointment, please give us 24 hours notice. Another person will be grateful for your time. Please call 719-265-0100.

Accepted Insurances

We participate in most major insurance plans and will be pleased to bill your insurance carrier on your behalf. Copays and deductibles are the responsibility of the patient. Because insurance plans vary greatly, it is your responsibility to contact your insurance carrier to determine eligibility, copay, deductible and referral requirements prior to your appointment.

Copays and Deductibles
If a copay is required by your insurance, it is to be paid prior to seeing our provider. We are required by your insurance to collect the copay at the time of service. Remember, insurance companies often apply different copay amounts for specialists. Please come prepared to pay this amount. Many insurance policies have a yearly deductible that must be met by the patient before the insurance will pay their percentage. Any unmet deductibles must be paid at the time of service for our surgical appointments.

Referrals
Some health insurance policies allow you to make an appointment directly with a specialist while others require a referral from your primary care physician. Other insurance companies only require a note from your primary care physician. If you have questions regarding referral requirements, please contact your insurance carrier. Individual policies vary greatly.

Secure Patient Portal

We are excited to offer you electronic access to your personal health information through our patient portal. This portal will allow you to update your medical and surgical history, pharmacy information, and medication list. You will also have access to summaries of your office visits and information about your diagnoses and treatments.
